The message shown also mentions TQAs and comments. I wonder if your file contains comments? Could you check if there's any and if so, remove them and then try again?

If it still does not work I would suggest aligning the reviewed file with the original source document - it should be quite quick as their structure should be the same.

Greta Magyar thank you! You were correct. There was one comment I had forgotten about. The Comment tab was not showing (1) in parenthesis (instead, it was showing (0)), so I had forgotten about that comment.
