Custom backend APi is already possible by implementing custom application, here is basic documentation for it https://docs.rws.com/en-US/tridion-sites-10.1-main-documentation-1174622/bff-web-app-clients-to-access-management-1101614

For our cloud hosted customers such custom application can be run only on seprate box. The reason for it is security concern, running inside same CM application means using same application registered for CM in Access Management so may result to secirity issues.

But let me put this idea under reviw to collect more feedback.
